Terms of Use
Generations Medical Group LLC
Effective Date: September 27, 2026
Welcome to the website of Generations Medical Group LLC ("Generations Medical Group," "GMG," "we," "us," or "our").
By accessing or using this website, you agree to these Terms of Use. If you do not agree with these terms, please do not use this website.
Medical Disclaimer
Information provided on this website is for general informational and educational purposes and is not a substitute for individualized medical advice, diagnosis, or treatment.
Use of this website does not by itself establish a provider-patient relationship with Generations Medical Group or any of its healthcare providers.
A provider-patient relationship is established only after appropriate registration, consent, and medical evaluation in accordance with applicable law and Generations Medical Group's clinical procedures.
Do not disregard professional medical advice or delay seeking medical care because of information you have read on this website.
Emergencies
Generations Medical Group's website and telehealth hormone-care services are not intended for medical emergencies.
If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Do not use website forms, email, text messaging, or routine telehealth scheduling to obtain emergency medical care.
Telehealth Services
Generations Medical Group provides certain healthcare services through telehealth.
Patients must provide accurate information about their identity, medical history, medications, symptoms, health conditions, and other information reasonably necessary for medical evaluation and treatment.
Patients must be physically located in a jurisdiction where the treating provider is legally permitted to provide care at the time healthcare services are delivered.
Availability of telehealth services may depend on provider licensure, clinical appropriateness, technology, and applicable law.
An in-person examination, additional testing, referral, or care from another healthcare provider may be recommended when clinically appropriate.
Hormone Evaluation and Treatment
Scheduling and paying for a hormone evaluation does not guarantee that hormone therapy, testosterone replacement therapy, medication, or any particular treatment will be prescribed.
Treatment decisions are made by a licensed healthcare provider based on the patient's medical history, symptoms, clinical evaluation, laboratory findings when appropriate, potential risks and benefits, and professional medical judgment.
A provider may determine that hormone therapy is not medically appropriate or that additional evaluation or testing is required before treatment can be considered.
Initial Hormone Evaluation
The current fee for the initial hormone evaluation is $150.
The initial evaluation includes a scheduled 30-minute telehealth consultation with a licensed medical provider.
The $150 fee is payment for the medical evaluation itself and does not guarantee eligibility for treatment or a prescription.
Laboratory testing, medications, and other services are not included in the initial evaluation fee unless expressly stated otherwise.
If a patient is eligible for treatment and chooses to enroll in Generations Medical Group's annual hormone care membership, the $150 initial evaluation fee will be credited toward the annual membership fee under the program terms in effect at the time of enrollment.
Annual Hormone Care Membership
Generations Medical Group currently offers an annual hormone care membership for eligible patients who choose to enroll.
The current annual membership fee is $1,800.
When the $150 initial evaluation credit is applied, the remaining enrollment balance is $1,650.
Enrollment is optional. Completing an initial evaluation does not require a patient to enroll in the annual program.
The services included in the annual membership will be described to the patient before enrollment and may include scheduled telehealth follow-up care, review of laboratory results, clinically appropriate treatment adjustments, ongoing monitoring, and provider support as described in the applicable program agreement.
Laboratory testing and medications are not included in the $1,800 annual membership unless expressly stated otherwise.
The annual membership does not guarantee any particular medication, dosage, laboratory result, or treatment outcome.
Separate membership terms, including applicable renewal, cancellation, and refund provisions, may apply and will be disclosed before enrollment.
Payments
Online payments may be processed through third-party payment processors, including Stripe.
By submitting payment information, you represent that you are authorized to use the selected payment method and authorize the applicable charge.
Patients are responsible for reviewing the price and applicable terms before completing a payment.
Cancellations and Refunds
Cancellation, rescheduling, and refund eligibility are governed by the policy disclosed to the patient at or before purchase or scheduling.
The initial evaluation fee compensates Generations Medical Group for a professional medical service. Once the medical evaluation has been provided, the evaluation fee is generally not refundable except where otherwise required by law or expressly provided by Generations Medical Group.
Additional cancellation and refund terms may apply to appointments, annual memberships, laboratory services, medications, or other services.
Please review the applicable Cancellation & Refund Policy before purchasing or scheduling services.
Laboratory Testing and Medications
Laboratory testing and prescription medications may be recommended when medically appropriate.
Unless expressly stated otherwise, laboratory testing and medications are separate from Generations Medical Group's initial evaluation and annual membership fees.
Costs may vary based on the laboratory, pharmacy, medication, testing ordered, and other factors.
A prescription is never guaranteed.
No Guarantee of Results
Individual responses to medical treatment vary.
Generations Medical Group does not guarantee specific results, symptom improvement, laboratory values, eligibility for treatment, or any particular medical outcome.
